Practioner on the Renal Team who can give you lots of useful practical help and
advice.

What does a Specialist Renal Practitioner do? The
Specialist Renal Practitioner's aim is to help you live, as much as possible, a
full, active and independent life in your everyday activities.
The
Specialist Practitioner will begin by talking with you (and your family) to
understand the difficulties you may be having - these may be financial,
psychological or emotional. Together you will be able to discuss and plan ways
of easing or resolving these difficulties to enable you to live as normal a
life as possible.
The Specialist Practitioner can see you on the Renal
Unit (RCH or WCH) or at home as needed.
The Specialist Practitioner
works closely with the members of the renal team and would seek their advice
and support as appropriate whilst helping you.
The Specialist
Practitioner may be able to help you and your family with the following
:-
- Providing equipment at home to help you manage
daily tasks
- Advising you on the benefits you may be
entitled to claim and helping you with benefit application forms and
appeals
- Help to keep you as independent as
possible
- General Support for you and your
family
- Charitable help for "extras" like holidays,
special home appliances etc.
- Advice and Information
- Encouraging you to keep as well as possible,
physically and emotionally, and helping to cope with long term
illness
